Form a list of 3-5 references that the potential employer can contact to discuss your abilities, work ethic, history, etc.
Place them in order of who you feel will provide the best reference for you, considering also who that person is. In other words, while your friend of 5 years may provide the best account, it will mean more coming from your supervisor of the last 2 years.
You can include personal references (they should not be family members), but your professional references should comprise over 50% of the list.
